Introduce text fields to scanned forms...

This is probably a dumb question, but I'm wondering if there is a software solution already created (or in the works) that allows me to scan a form, and introduce fields into the form in order to print it out.

I don't want to do this manually every week (like with Gimp) as I would rather keep my years worth of records within a relatively small footprint.

I'm looking for a solution where the actual form is saved only once, and text fields are applied to it, tagged and then the records are saved in a database that can then be be printed out in the future.

Has this already been done? If not, any pointers on how to go about creating an app that will do it? (it would definitely be a learning experience, but I'm not scared! :P )

way over my head
but have a look at OpenOffice, you might be able to set the scanned picture in the background and then drop fields over areas of the page so it looks like the form is being used.

Hi,

Scribus does a nice job in creating PDF forms. I haven't tried inserting a background picture, but hey, why not?
